Tab 3 of the Personal Health Portfolio is a two-pocket divider containing an Emergency Contacts form and an Appointment Log

Emergency Contacts – Your personal health portfolio includes your personal emergency contacts like sons, daughters, brothers, sisters and other family members who you would want contacted in the event of a medical emergency.  In addition to personal contacts, your health care plan information and pharmacy contacts are also listed on this form.

Appointment Log – The appointment log is used to track your medical appointments and insurance co-pays.  The benefit of this form is that it helps you to remember when you last saw a particular physician, and it helps you to keep track of out of pocket expenses.
