Tab 4 of the Personal Health Portfolio is a two-pocket divider where one pocket is empty allowing storage of lab/test results.  The other pocket contains three advanced health directive forms.

Lab / Test Results – A divider pocket is provided in the Personal Health Portfolio for you to store, and easily retrieve, lab and test results that may be needed by other health care professionals, or medical facilities.

Advanced Health Directives–For your convenience, the Personal Health Portfolio
contains a Living Will Declaration form, a Health Care Surrogate form, and a Medical Power of Attorney form. These forms are not only important documents for anyone with a serious medical condition, but are
important documents for all adults. The completion of these forms helps ensure your wishes regarding end-stage treatment resulting from accidental injury or illness are carried out.
